REPORTING SCHEDULE FOR B. PHARM. LATERAL ENTRY

The following original documents are required at the time of reporting for admission into Bachelor in Pharmacy Lateral Entry (B. PHARM. L.E.).

1. 10^th Pass certificate.
2. 10+2 certificate or its equivalent exam.
3. 10+2 Mark Sheet.
4. D. Pharm. Pass Certificate and D. Pharm. Part – I & Part – II Mark Sheets
5. College leaving Certificate/Transfer Certificate from the institution last studied/attended. (Original)
6. Conduct certificate issued by the Principal/ Head of the Institution last studied / attended. (Original)
7. Migration Certificate where applicable. (Original)
8. SC/ST/OBC/SEBC/PH etc. if applicable
9. SIX recent passport size and SIX stamp size color photographs
10. Attested copies of all certificates from Sl. No. 1 to 6 – 2 sets.
11. OJEE Rank Card.
12. OJEE Final Allotment letter

1. 10^th Pass certificate.
2. 10+2 certificate or its equivalent exam.
3. 10+2 Mark Sheet.
4. D. Pharm. Pass Certificate and D. Pharm. Part – I & Part – II Mark Sheets
5. College leaving Certificate/Transfer Certificate from the institution last studied/attended. (Original)
